Schoen Klinik Bad Bramstedt
The Medical-Psychosomatic Clinic Bad Bramstedt belongs to the Schoen Kliniken group and offers a specialized treatment program for personality disorders.
Treatment Concept
Section titled “Treatment Concept”Personality disorders are combined interaction and self-regulation disorders that begin at the latest in adolescence/puberty. The behaviors that emerge represent, at least at that point, a kind of protective or survival strategy intended to prevent recurring painful experiences of disappointment and overwhelm. As such “actually appropriate” behaviors, they were acquired in previous years as a learning and adaptation process to difficult and not infrequently traumatic life conditions.
The decisive problem for therapy, however, is the resulting lack of coping strategies for many everyday situations and the stress that follows.
Within the framework of standard and specific behavioral medicine treatment approaches, improvements are sought in:
- Situational mindfulness
- Stress tolerance
- Emotional regulation
- Relationship management skills
- Social and assertive competence
- Problem-solving skills
- Anxiety management
- Regulation of eating behavior
- Building positive activities
- Correcting unrealistic core beliefs
Target Problems of Treatment
Section titled “Target Problems of Treatment”- Low frustration tolerance, increased interpersonal sensitivity to perceived slights
- Lack of self-assertion and assertiveness skills
- Insufficient problem and conflict resolution abilities
- Inadequately differentiated capacity for tension regulation
- Situationally inappropriate self-perception, unstable sense of identity
- Deficient or unstable emotional regulation, inadequate impulse control
- Dissociative phenomena, especially depersonalization (“flight inward”)
- Greatly heightened need for affection and recognition that burdens others
- Insufficiently controllable self-harming and other-harming behavior
- Self-damaging substance abuse
Therapy Strategies
Section titled “Therapy Strategies”At the center of the cognitive-behavioral treatment concept are:
- Skills training
- Behavioral development and problem-solving training
- Exposure exercises
- Specific cognitive techniques
The methods are based on:
- Dialectical Behavior Therapy (DBT) by Linehan
- Interactional strategies by Grawe
- Cognitive methods by Beck
Pharmacological therapies are often helpful and therefore necessary supplements.
Therapeutic Framework
Section titled “Therapeutic Framework”Individual therapy plans and long-term therapy goals and agreements, including contracts and clearly discussed consequences, provide a firm therapeutic framework. The therapists accept each patient in their “self-protective” strategies, ensure very high transparency of the therapeutic approach regarding all change steps, and are open to questioning.
The open approach to and thorough exploration of the diagnosis “personality disorder” and an individual understanding of the disorder has proven very effective — embedded in the highly accepting ward atmosphere created by the homogeneous patient population, most patients experience this mutual process as clarifying, relieving, and motivating.

Conditions and Ward
Section titled “Conditions and Ward”The personality disorders department consists of 3 wards with a total of 74 beds, including 48 single rooms. All beds are occupied by patients with this diagnostic group, which on one hand allows very specific and focused work on central problems, and on the other hand creates a very supportive atmosphere on the wards.
Treatment Team
Section titled “Treatment Team”The treatment team for each ward includes:
- 2-3 clinical psychologists
- 1-2 physicians
- 1-2 co-therapists
- 1 movement therapist
- 1 art therapist
- The senior physician
Therapy hours: 8:30 AM to 5:00 PM
Psychiatric nursing is generally not provided. Care outside therapy hours takes place — off the ward — at the medical center through on-call medical, psychological, and nursing staff.
Treatment Duration
Section titled “Treatment Duration”Treatment duration is typically 12 weeks with a minimum of 6 weeks, and always with the possibility of readmission, as the concept is designed to be fractionated.
Prerequisites
Section titled “Prerequisites”A preliminary interview with the senior physician team leader takes place before admission. This is arranged after submission of registration documents (primarily a personal and medical report).
Cost coverage for inpatient treatment is available through all insurance carriers; hospital admission usually occurs under Section 39. Since this is a very specific treatment offering, supra-regional admission is also possible.
